Background: A Couple’s Struggle with Hope and Fear
A couple undergoing in-vitro fertilisation (IVF) approached us with a crucial challenge. Before proceeding with their embryo transfer, the patient was advised to remove fibroids that could interfere with her chances of successful implantation and pregnancy.
Fibroids, also known as uterine leiomyomas, are benign growths in the uterus that can sometimes cause heavy bleeding, pelvic pain, or fertility complications. In women preparing for IVF, fibroids that distort the uterine cavity can significantly reduce implantation rates or increase miscarriage risk.
Before visiting Nurturing Petals, the couple had already consulted several hospitals. However, they were repeatedly told that surgery might be highly complicated and that there was a risk of losing the uterus if excessive bleeding occurred during the procedure. For a woman already struggling with infertility, this possibility was devastating.
By the time they reached our clinic, they were anxious, emotionally drained, and uncertain if they would ever be able to pursue parenthood.
Our Approach: Empathy Before Everything

- Anaesthetic risk – The patient had a high body mass index (BMI) and a short neck, both of which posed an elevated risk for general anaesthesia and intubation.
To move forward safely, our team needed a clear plan supported by accurate data and multidisciplinary preparation.
Step 1: Accurate Diagnosis through 3D Fibroid Mapping
The first step was to eliminate the confusion created by the earlier scans. We recommended a 3D fibroid mapping at a trusted diagnostic centre. Unlike conventional ultrasound, 3D mapping provides a detailed three-dimensional view of the uterus, clearly showing the exact size, number, and position of all fibroids.
This investigation revealed multiple fibroids, some embedded within the uterine wall and one pressing close to the uterine cavity, the one most likely to affect implantation. Having this information allowed our team to strategically plan the surgical approach, ensuring all fibroids affecting fertility would be removed while keeping the uterus intact.
Step 2: Safe Anaesthesia Planning
The next challenge was the patient’s anaesthetic risk. Because of her high BMI and short neck, intubation could be difficult. To mitigate any potential risks, we arranged a pre-anaesthetic assessment with a senior anaesthesiologist.
This evaluation included:
- Airway examination and simulation for intubation strategy
- Assessment of cardiopulmonary function
- Discussion with the surgical team regarding positioning and duration of surgery
With a well-documented anaesthesia plan and safety measures in place, we ensured that the patient would receive care that was not just effective but also meticulously safe.
Step 3: Laparoscopic Surgery – Precision with Preservation
The patient was discharged on the same day, with clear post-surgery care instructions. Recovery was uneventful, and the patient reported no major discomfort. Both the patient and her partner felt reassured by the timely diagnosis, clear communication, and professional care they received at our centre.
Recovery and Outcome
Post-surgery, the patient’s recovery was smooth. She was able to walk, eat, and sit up on the same day of the surgery — something rarely possible with traditional open methods. Pain was minimal, and no postoperative complications occurred.
By the next evening, she was discharged with post-operative care instructions and guidance on when she could resume her IVF treatment. Within a few weeks, she was physically fit and emotionally ready to continue her fertility journey. When the couple left the hospital, they were visibly relieved. What began as a frightening situation had turned into a story of hope and healing. The surgery not only preserved her uterus but also gave her the confidence to move forward in her dream of becoming a mother.
