Polyhydramnios in Pregnancy-case study

An Expert Case Report from Nurturing Petals Hospital & Fertility Centre, Andheri, Mumbai

Polyhydramnios (excess amniotic fluid) is a high-risk pregnancy condition that requires careful monitoring and timely decision-making. At Nurturing Petals Hospital & Fertility Centre, Andheri, Mumbai, we manage such pregnancies with a structured, safety-first approach to ensure the best possible outcome for both mother and baby.

This case highlights how early diagnosis and planned elective Caesarean section can help prevent serious complications associated with polyhydramnios.

Case Summary

A primary gravida, married for three years, was on regular antenatal follow-up at our centre in Andheri, Mumbai. All routine antenatal investigations and ultrasound scans were normal during the early and mid-pregnancy period.

From 28 weeks of gestation, ultrasound examination revealed increased amniotic fluid volume, consistent with a diagnosis of polyhydramnios.

Evaluation & Diagnosis

As gestational diabetes mellitus (GDM) is the most common cause of polyhydramnios, the patient underwent detailed evaluation:
– Oral Glucose Tolerance Test (OGTT): Normal
– No fetal structural anomalies detected
– No evidence of infection or Rh incompatibility

After ruling out common causes, the patient was diagnosed with idiopathic polyhydramnios.

Why Polyhydramnios Is a High-Risk Condition

Polyhydramnios can lead to several maternal and fetal complications, especially during labour.

Maternal risks:
– Uterine over-distension
– Preterm labour
– Placental abruption
– Postpartum haemorrhage (PPH)

Fetal and intrapartum risks:
– Umbilical cord prolapse
– Malpresentation of the baby
– Sudden rupture of membranes
– Fetal distress
– Increased operative delivery rates

Among these, cord prolapse is one of the most serious and unpredictable complications.

Treatment Planning & Counselling (Under the care of Dr Milind Chauhan)

The patient was counselled in detail by Dr Milind Chauhan regarding the nature of polyhydramnios, possible complications during vaginal delivery, and available delivery options.

Considering the excess amniotic fluid, risk of cord prolapse, and the need for a controlled delivery, a joint decision was taken with the family to proceed with a planned elective Caesarean section at 38 weeks of gestation.

Planned Elective C-Section & Outcome

The elective C-section was performed in a controlled hospital setting at Nurturing Petals Hospital & Fertility Centre, Andheri, Mumbai, with all precautions in place.

The surgery was uneventful. A healthy baby was delivered. Both mother and baby remained stable post-operatively, and recovery was smooth with no complications.

Key Learning Points

– Polyhydramnios requires close antenatal surveillance
– Even idiopathic polyhydramnios carries significant risks
– Planned elective C-section can prevent emergencies like cord prolapse
– Individualised care improves maternal and neonatal outcomes

When Should Pregnant Women Seek Medical Advice?

Pregnant women should consult a gynecologist if they experience rapid increase in abdominal size, breathlessness or abdominal discomfort, tightness or pain in the abdomen, or changes in fetal movements.

Early evaluation by an experienced gynecologist in Andheri, Mumbai can prevent complications.

Conclusion

Polyhydramnios is a condition that requires expert monitoring and timely intervention. With appropriate evaluation, counselling, and planned delivery, excellent outcomes can be achieved even in high-risk pregnancies.
