Pregnancy is often a journey filled with excitement, dreams, and anticipation. However, for some couples, that journey can unexpectedly take a painful turn.

One of our patients experienced a missed miscarriage (missed abortion) during her first pregnancy. The diagnosis came as a shock because she had no significant symptoms and believed everything was progressing normally.
Like many couples, they were devastated and anxious about whether they would ever be able to have a healthy baby.
Today, we are delighted to share their story of hope, perseverance, and a successful outcome.

What Is a Missed Abortion?

A missed abortion, also known as a missed miscarriage, occurs when the pregnancy stops developing, but the body does not immediately recognize the loss.

Unlike a typical miscarriage, there may be:

  • No bleeding
  • No abdominal pain
  • No warning signs

The diagnosis is often made during a routine ultrasound when:

  • No fetal heartbeat is detected
  • The embryo has stopped growing
  • The pregnancy is no longer viable despite the absence of symptoms

Many women are understandably shocked because they may continue experiencing pregnancy symptoms even after the pregnancy has stopped developing.

Early pregnancy loss is unfortunately common and occurs in approximately 10% of clinically recognized pregnancies. Nearly half of first-trimester miscarriages are related to chromosomal abnormalities that occur by chance.

The Emotional Impact

For most couples, a miscarriage is not merely a medical event.

It represents the loss of dreams, expectations, and future plans.

Common questions include:

  • Why did this happen?
  • Will it happen again?
  • Do I need treatment before trying again?
  • Will I ever have a healthy baby?

These concerns are natural and deserve careful counselling.

An Important Counselling Point for Couples

One of the most important aspects of miscarriage care is counselling.

As doctors, it is our responsibility to reassure couples that a single miscarriage is usually an isolated event.

Most women who experience one miscarriage go on to conceive naturally and have completely healthy pregnancies without requiring extensive treatment.

Current guidelines do not generally recommend a complete recurrent pregnancy loss workup after a single miscarriage unless there are specific clinical concerns or additional risk factors. Investigations are usually considered after recurrent pregnancy losses.

In fact, most first-trimester miscarriages occur because of chromosomal abnormalities that happen by chance and are not caused by anything the couple did or failed to do.

However, every patient is unique.

Depending on:

  • Maternal age
  • Medical history
  • Ultrasound findings
  • Previous obstetric history
  • Family history
  • Clinical suspicion

certain investigations may be recommended to rule out potentially treatable causes.

The goal is not to create anxiety but to optimize the chances of a healthy future pregnancy.

A Detailed Evaluation

After recovering from the missed miscarriage, the couple underwent extensive counselling and preconception planning.

As part of the evaluation, the following investigations were performed:

Hormonal Assessment

  • Thyroid Profile (TSH)
  • Blood Sugar Assessment (HbA1c)
  • Hormonal evaluation when indicated

Autoimmune Assessment

  • Antiphospholipid Antibody (APLA) Testing
  • Lupus Anticoagulant
  • Anticardiolipin Antibodies

Uterine Assessment

  • Pelvic Ultrasound
  • Evaluation of uterine anatomy

General Health Assessment

  • Complete Blood Count
  • Nutritional Assessment
  • Screening for associated medical disorders

During this evaluation, the patient was found to be APLA positive.

Understanding APLA

Antiphospholipid Antibody Syndrome (APS) is an autoimmune condition that can affect placental development and pregnancy outcomes.

Among the many causes investigated in women with recurrent pregnancy loss, APS remains one of the few clearly established and treatable conditions associated with miscarriage and adverse pregnancy outcomes.

Identifying this condition allowed us to formulate a specific treatment plan before the next pregnancy.

Planning the Next Pregnancy

The couple was counselled regarding the findings and available treatment options.

Once pregnancy was confirmed, she was started on:

  • Low Molecular Weight Heparin (LMWH)
  • Low-Dose Aspirin
  • Progesterone Support
  • Folic Acid Supplementation
  • Nutritional Support
  • Close Antenatal Monitoring

Women with confirmed APS may benefit from anticoagulation-based treatment during pregnancy under specialist supervision.

She was followed regularly throughout pregnancy with:

  • Clinical examinations
  • Blood investigations
  • Growth monitoring

Ultrasound assessments

A Smooth Pregnancy Journey

With appropriate treatment and regular follow-up, the pregnancy progressed beautifully.

The baby’s growth remained appropriate throughout pregnancy.

Regular scans were reassuring, maternal health remained stable, and both mother and baby continued to do well.

Each antenatal visit brought renewed confidence and hope to the family.

The Final Chapter

At 38–39 weeks of gestation, labour was induced.

Despite adequate efforts, labour failed to progress satisfactorily.

After careful assessment and discussion with the family, an emergency cesarean section was performed.

Soon afterward, the couple welcomed a healthy and beautiful baby into their arms.

The joy on their faces was unforgettable.

What began with heartbreak and uncertainty ended with laughter, tears of happiness, and a new beginning.

Lessons From This Journey

Most women who experience one miscarriage will go on to have successful pregnancies.

Not every patient requires extensive investigations, but selected patients may benefit from targeted evaluation based on their history and risk factors.

Conditions such as Antiphospholipid Antibody Syndrome can be identified and managed effectively, significantly improving pregnancy outcomes.

A pregnancy loss can be devastating, but it is often only one chapter in a much larger story.
